I love the Romance of the Three Kingdoms series myself, but the way CK2 is designed I'm not sure if it'd translate over very well. For one the de jure system does not have a very good equivalent in the setting/time period. It'd also be a bit more difficult to translate the more meritocratic elements.
It would make sense to put 3/4 and 5/6 baronies in a single county in the most populated provinces and 1/2 or 2/3 in the most sparsely populates ones.
