In January 2023, AURORA was in the studio working on "so many babies". In March, it was announced she will add "The Earth" to songwriting credits, so that royalties are diverted to environmental causes. Aurora said:

After one of the last shows of her The Gods We Can Touch Tour, she posted on social media she's going to make a new "baby". Geir Luedy, one of Aurora's managers, said in a podcast recorded circa October 2023 that Aurora is working on a new record.

On November 8, 2023, Aurora released the single "Your Blood". On January 18, 2024 she released a second single, "The Conflict of the Mind". On March 20, 2024, she released "Some Type of Skin".

The album was officially announced on March 28. Before the reveal, several websites showed an incorrect release date (June 14).

On April 18, a remix of the album track "A Soul with No King" (featuring Nature) was released in collaboration with Sounds Right, EarthPercent and the NGO Museum for the United Nations.

More information about songs on the album were shared in promotion of her tour in Europe and America.

According to the promo text for the album, in April 2022, Aurora read a letter that changed her life. It was co-written by indigenous activists, titled "We Are the Earth", and called for a revolution: a collective response to global warming – to "heal the land". They described being connected to the land "through our hearts", and the earth as "the heart that pulsates within us." The letter led Aurora to consider a question: what happened to the heart? And she began studying books on human anatomy, wanting to understand when and why Western culture lost touch with the deeper purpose of our most vital organ. What Happened to the Heart? is a journey from weakness to strength, from self-destruction to self-healing, a poignant and introspective musical journey that explores the loss of spiritual connection in modern society, the healing power of vulnerability, and the call to ignite change through reuniting the heart with politics and personal growth.

Aurora described this album as her most personal and cathartic, adding:
