What is the Child Health Booklet? Since 1984, every Singapore child grows up with their very own Child Health Booklet (CHB), also affectionately known as the “baby book”. This booklet is provided to every baby at time of delivery in hospitals in Singapore.

The Child Health Booklet (CHB) is bestowed upon every newborn in Singapore, serving as a personalized health repository. From birth data to developmental milestones , this booklet encapsulates your child’s health journey, making it easily accessible for parents and children alike.
You will be able to view and update information like your child's height and weight, developmental milestones and birth information. For school-age children, you will also be able to view your child's health records, oral health and immunisation records, if available.
Prior to each visit, please take time to fill in the pre-screening developmental questionnaire in your child's health booklet. Your information will help us understand your child's progress and the areas to focus on during the clinic visit.
