The National University Health System (NUHS) Pharmacy manages pharmaceutical services to our patients seeking care in our polyclinics under the National University Polyclinics (NUP). We aim to deliver safe and efficacious drugs and pharmacy services to patients in a cost-effective manner.
Form is applicable for home delivery orders or for order-&-pick-up at NUH pharmacy. Home delivery: 5 working days Order-&-pick-up: 3 working days Please keep a screenshot of your order after completion of the form for verification.
How Can You Order? You may submit your medication delivery request via the NUHS app or call the NUHS Contact Centre at 6908 2222. Kindly ensure that all information submitted is accurate for order processing.
