Feb 1, 2013 · The SOOF is the sub-orbicularis occuli fat (pad). This fat compartment is an important part of the youthful volume of the midface. It is lifted typically through a lower blepharoplasty incision (lower eyelid lift approach). This works well in some patients, but can cause unexpected changes in the shape or tension of the lower eyelid.

The implants for this area that are available off the shelf primarily create very unnatural midface volume so I take out a lot of these. SOOF stands for suborbicularis oculi fat pad. There is a lot of misunderstand of this structure. The SOOF is actually part of the malar fat pad.

Understand that soof lift as a blephoroplasty option is optimal because it addresses the juncture of the lower eye and upper cheek but that this alone will not restore descending of your cheeks. I believe this procedure would be optimal in combination with an upper midface cheek lift.

The SOOF lift is a great procedure and is often a very effective means of softening the lower eyelid - cheek junction. In my opinion, a SOOF lift works best when combined with a transposition lower eyelid blepharoplasty where the orbicularis retaining ligament is released and fat from the lower eyelid region is redraped over the orbital rim.

Traditional blepharoplasty dissections stop at the lower orbital rim, whereas extended blepharoplasties "extend" below the the rim to release what is known as the orbital malar ligament (OML).
