The CHAS Online Application is an e-Service which provides the public with a convenient and faster way to apply for CHAS. Singapore Citizens aged 21 years and above may access this e-Service with their Singpass accounts and submit the application on behalf of their household members.
You can apply for CHAS via the online application by using this link. Alternatively, you can also pick up a CHAS application form at any Community Centre or Club (CC), Community Development Council (CDC), Public Hospital or Polyclinic or download the application form by using this link [PDF, 631KB].
To enjoy subsidies under CHAS, submit an application on www.chas.sg/apply. All Singapore Citizens are eligible to apply. The colour of the CHAS card indicates the subsidy tier that cardholders are entitled to, namely the CHAS Blue, Orange or Green tier.
